INCIDENT REPORT P-130x-008
01: IMMEDIATE CONCERNS
All personnel conducting experiments involving the hzSuit must first verify the hermetic integrity of the apparel. All volunteers must be monitored, guided and accompanied throughout all stages of preparation, including changing and cleaning. The following report details the actual and intended sequence of events surrounding incident P-130x-008, occuring on [DATE REDACTED] during an materials observation test of substance P-130x.

04: INTENDED SEQUENCE OF EVENTS
P-130X M.O.04 was a routine materials observation test conducted to study the properties of the synthesized P-130x.
The objective for the day was to study the autonomous properties of the substance in contact with a human body, and observe the tactile response of the material.
Subjects protected within hzSuits were to be lowered into a pool of P-130x and suspended for a minimum duration of fifteen minutes before being hoisted out and decontaminated.
Each volunteer was to produce a report detailing the observed behavior of P-130x whilst placed in near-direct contact.
05: SAFETY PRECAUTIONS TAKEN
All Pilco Safety Protocols were observed. Special considerations and provisions were made for the following:
- Three freshly molded hzSuits were prepared for three volunteers as per Pilco Safety Protocol. This is to ensure that no direct contact is made with P-130x.
- A freshly-spun nanomesh net must be positioned over the vat as a precaution against falls and accidental immersion.
06: ACTUAL SEQUENCE OF EVENTS
At 1103, Subject #1 entered the test chamber after donning her hzSuit. Subject #1 experienced mild difficulty walking due to excessive lubrication inside the suit, but testing was conducted successfully and without incident.
At 1137, Subject #2 entered the test chamber. She observed an unusual stickiness of the hzSuit, but no further action was taken to resolve this. Upon lowering into P-130x, a breach in her hzSuit became apparent and she was quickly removed from the material.
The nature of the hzSuit prevented the volunteer and safety handlers from quickly accessing the P-130x contamination to restrict its spread.
The hzSuit was cut open as quickly as possible, but the substance had already completely coated the subject.
P-130x M.O.04 was then terminated by Project Lead Olivia Hobb at 1148.
07: CAUSE OF FAILURE
The direct cause of failure stems from the compromised hzSuit. A minute breach in the material allowed P-130x to reach the volunteer and act outside of the control of the test conduct.
Study of data logs and observation camera footage show that Volunteer #1 had damaged her hzSuit due to a slip outside of the test chamber. Her hzSuit was then returned to the wrong bin, causing Volunteer #2 to mistakenly retreive the damaged hzSuit instead of her assigned unit.
No personnel was present to correct this.
08: CURRENT SITUATION
Volunteer #2 has been designated S-130-008. Immediately following the events of the test incident, S-130-008 was observed to have been completely sealed within a thin layer of P-130x.
The material behaves like rubber but appears to be dynamic, and seems to move within its layer and around the subject. It is invasive, and has penetrated the subject's orifices. It is unclear the extent to which P-130x has permeated its host.
S-130-008 is responsive and conscious, though she appears to have lost the need to breathe. She is unable to talk but can respond using non-verbal methods of communication. Attempts to interact with her are slow as P-130x seems to distract her from active thought. It is not clear from the outside what triggers the movement of the substance.
It's not immediately clear the implications this has on the subject. Whilst the accident is unfortunate, it may be worthwhile to take the oppurtunity to study the deeper impact that this has on the human body.
09: RECOMMENDATIONS
A revision of Pilco Safety Protocol should be considered, to allow for guidance and observation of volunteer personel through use of mission-critical apparatus. Modesty is a personal luxury that we cannot afford considering the nature of our work.
To prevent further contamination, S-130-008 is to be spun into a nanoweb suit. Maddie's nanoweb mesh has proven to be effective in containing all variants of P-Zero, and until more conclusive action can be taken to release S-130-008's host, measures must be taken to prevent accidental spread of P-130x.
