By the mid-eighteenth century, the British Navy faced a growing shortage of manpower. Its supply of womanpower, however, was more than sufficient.

Initially hired out of necessity, the role of midwife-surgeon or gynecology-surgeon had become one of the most well-paid professions open to women and had supplanted traditional barber-surgeons. They became supremely skilled at their art, with a level of proficiency not rivalled until the cosmetic surgeons of the twenty-first century.

Feminization of seamen became commonplace. In some cases, this was purely due to a lack of knowledge, misunderstanding of what constituted normal male anatomy amongst the female medical practitioners. Other times, a castration might be from the simple, accidental slip of a scalpel on a rocking boat. But there does also appear to have been plenty of women-surgeons of the time who exhibited a profound glee in demasculinizing shiploads of men; no doubt a visceral reaction to the oppressive patriarchal society under which they lived.

Recorded evidence of resistance from the crews is startlingly scant. In fact, ships with a handful of altered sailors were notably more harmonious. Fears that such crews would descend into debauchery were only partially justified. Many captains found that the pseudo-heterosexual relationships that formed aboard these vessels were, in their view, marginally more acceptable than open homosexuality.

The most pressing concern was the exodus of sailors from naval service. In the woman-starved colonies of the Americas, many of these individuals found they could greatly improve their fortunes by becoming the mistresses or wives of wealthy settlers. While in the Far East, they were sought as unique, exotic beauties, earning many times a sailors wage selling their bodies as courtesans in the ports of Manila and Macau.
