Patient was conducting underwater maintenance on an oil pipeline. At approximately 19:45, Patient surfaced in extreme distress claiming to have been attacked by a marine organism. Patient was assisted from the water, whereupon a large fish-like animal was found latched to his lower body, having engulfed the patient to his waist, its teeth penetrating the diving suit. Patient was transported to us via helicopter, arriving within 90 minutes of the incident.

Clinical Findings:

The organism completely encircles the pelvis with its mouth. Lacerations and abrasions on the lower abdomen and pelvis. Unable to verify the status of the legs. Patient is understandably distressed and in considerable discomfort, though less pain than might be expected - he reports his entire lower body is numb - I'm unsure whether that is good or bed given the circumstances. I have assessed the integrity of his spine since the creature's teeth are mere centimeters away. The spine is thankfully uninjured. It may be possible that the creature's mouth is secreting some sort of analgesic agent.

The organism is approximately two meters long, covered in iridescent green scales, with a single, vast, unblinking eye on its upper surface. It resembles no fish that I, nor the oil riggers, nor coast guard pilots who attended with the patient recognize.

Patient commenced on pain management and transfusion for blood loss. Any attempt to pry the organism off results in the creature biting down harder. Given the proximity of its teeth to his spine, I am opting for a conservative approach for now, at least until more specialist consultation can arrive, in the hopes the organism will tire or suffocate on land and release its grip. I am sending the patient for an urgent CT scan to assess the extent of any damage.

CT Findings:

Extensive abnormalities of the legs and pelvis. The musculature, soft tissues, and vascular structures of the lower legs appear grossly deformed, absent, or undergoing a process of liquefaction. Segmentation of the major bones. Total dissolution of the smaller bones of the feet and ankles. The organism's digestive tract is contracted tightly around the remaining flesh and, in part, partially integrated with it. Vascular structures can be observed extending from the fish's body, threading through the patient's muscles to penetrate his femoral and iliac arteries. The patient's genitals appear atrophied, borderline vestigial, within the thing's mouth.

What may be a feeding proboscis extends from the inner throat of the fish, penetrating the patient's digestive tract via the anus up to the descending colon, at which point it becomes impossible to say where one ends and the patient's anatomy starts. Colon is bloated with viscous liquid and shows atypical changes. It is not dissolved, showing coherent structures, but it doesn't look human either.

It is as if the organism is fusing with the patient on an anatomic level. At a guess, it's as if the fish is making itself an extension of the patient's digestive tract so that his waste flows into it. Could we be dealing with a parasite here? I don't know - I'm a radiologist, not a fisherman!

Recommendation: Given the involvement of such major arteries, I can see no way of removing this thing without the patient bleeding out, or without amputating everything from the waist down. But whatever you do, I suggest you do it fast!
