Patient: Dumas, Thomas
Diagnosis: Male Pseudocyesis, Testicular Hypogonadism, Gynecomastia
Doctor: Could you start by telling me your name?
Thomas: Thomas. Thomas Dumas.
Doctor: And tell me, would you consider yourself to be a man or a woman?
Thomas: Don't be daft, doctor, obviously I'm a man.
Doctor: And you were born a man?
Thomas: Yes, or course. I don't understand where these questions are going.
Doctor: I'm merely attempting to ascertain your mental state. Now, can you tell me where you are and what you're doing here?
Thomas: I uh... I... This is a maternity ward, isn't it? I'm... I'm here because of my baby. That must be it.
Doctor: I see. And this baby... where do you believe it to be exactly?
Thomas: In my womb.
Doctor: I see. Your... male womb. You acknowledge that you're a man and you also believe yourself to be pregnant. Now, think, Mr Dumas, doesn't something about that seem strange? Something doesn't add up there?
Thomas: ... Huh... No, you're right. That doesn't make sense. I can't be both.
Doctor: Very good...
Thomas: How silly of me. It's so obvious now. I must be a woman.
Doctor: Yes, very g- wait, what? No. No, that's not it at all.
Thomas: How else to explain my body?
Doctor: Mr Dumas, you are not a woman. And you certainly are not pregnant. You are on a mental health ward. You are currently suffering a delusional episode. We believe you have pseudocyesis, a delusion of pregnancy.
Thomas: Oh! Oh my god! Really?
Doctor: I'm afraid so. Yes
Thomas: Is that going to affect the fetus?!
Doctor: The fe-? What? No. No, there is no-
Thomas: Oh, thank god.
Doctor: Mr Dumas, there is no fetus. You are hallucinating, and your mind is making it real. It is rewriting your body chemistry. Your hormones are completely off the charts.
Thomas: What an insensitive thing to say to a pregnant woman.
Doctor: Oh, for goodness sake, Thomas. We are worried about you. Your wife is worried about you. Your delusion is warping your body, and your appearance is feeding back into your delusion. It has become a positive feedback loop and I fear, if you don't break out of it soon, you may well become trapped within it. Do you understand what I'm saying?
Thomas: ... But the baby is fine?
Doctor: *Sigh*
