Subject number thirty-one, aka Mrs. Jennifer Michaels, age twenty-nine. Came into a local urgent care complaining about chest pains. Nothing actually wrong with her heart, just a minor panic attack. A doctor referred her to a psychiatrist, a Dr. Tanner Pittman for evaluation should she decide to go.

Upon entering the office, subject thirty-one seemed confused and agitated. Upon questioning, she stated she was nervous about her diagnosis and confused as to the office. She then explained that the office seemed more akin to a vintage doctor's office than a modern psychiatrist's office. It was explained to her that most of it was originally a doctor's office that had closed many years ago and were still in the process of converting to a modern medical office. Subject seemed incredulous, but did not leave and agreed to met Dr. Pittman.

Upon her entering the prep room, she immediately realized that this was not normal procedure and attempted to leave. Sedative administered, and she became calm and compliant. Dr. Pittman then explained to her the procedure as she was put onto the operating table. With her unconscious soon after, the procedure could begin.

Procedure twenty-seven was a complete success. Subject number thirty-one's head and neck successfully removed and replaced with Dr. Pittman's head and neck. No complications, and all medical waste has been properly and discreetly disposed of. Dr. Pittman, aka patient six is awake, alert, and initial testing reveals no immediate medical concerns. Further testing, required. Next test: sexual functionality. Will monitor.
