How the hell, he thought, does a mistake like this happen? He came in for stomach problems and he leaves like this, his head grafted onto a woman's body! She had already left, angry as hell as hell, and now it was seemingly his turn.

How does this even happen, he wondered to himself. You can't possibly be this incompetent! There's no way you can be! You can't take someone in for stomach issues and accidentally put their head on a new body! You can't!

Wait, you can't. He stopped. This wasn't a mistake. Realization started to hit him. This was planned, or at least the operation was planned.
